As part of her diagnostic workup, ST underwent a dobutamine stress test. During the procedure, the echocardiogram revealed global changes, indicating significant stress on her heart. Further examination showed alarming ST elevations in leads 2, 3, and AVF on her ECG, confirming the presence of an inferior ST-Elevation Myocardial Infarction (STEMI). The heart alert was called immediately. For quite some time, I could see the fear overwhelming ST. Her respiratory rate was increasing and she was hyperventilating. She had an ascending level of chest pain and was heavily diaphoretic. In the flurry of the emergency, the cardiac team discussed administering nitroglycerin to alleviate ST’s chest pain. However, I voiced my concern, "I think we should reconsider the nitro," "It's contraindicated in right-sided myocardial infarction due to the risk of dropping the preload and causing hypotension." The team quickly adjusted the treatment plan and prepared ST for an urgent cardiac catheterization. Once in the cath lab, the cardiologists discovered a 100% occlusion in ST’s right coronary artery (RCA). A stent was promptly placed, restoring blood flow and stabilizing her condition. Later, as ST rested in her room, I took the opportunity to stop in and see her as I was on that unit providing care to another patient. I had the opportunity to provide post-procedural care and education. ST’s family was in the room visiting and providing emotional support. I asked her if she had a family history of coronary artery disease. She stated that she had long-standing history of mother and father having open heart coronary bypass surgery. Her children were present in the room visiting. I informed her and the family that genetic comorbidity was the leading factor in developing coronary artery disease and that her children, who were in 20’s should have these concerns on their radar. The patient was very grateful for my visit and the education provided.
